Important information about your visits

Appointments

Please be as specific as possible regarding your needs when scheduling so that an appropriate amount of time can be allotted for your visit. We strive to be on schedule and appreciate your timeliness as well. Late arrivals may need to be rescheduled.

Insurance & Billing

Although we will do the billing for insurance plans we're contracted with, each patient must be responsible for obtaining any required prior authorization or referrals. We do NOT file on secondary insurances unless you have Medicare.

Payment

We accept checks, cash, and major credit cards. No post-dated checks are allowed. There will be a $30 fee for any returned checks. Payment is expected at time of service for any co-pays, deductibles, or non-covered services.

Referrals

For many insurance plans, referrals must be obtained after consultation with your primary care physician and in accordance with your plan's requirements. Please verify with your insurance prior to your appointment.

Medication Refills

Refills of medication may be requested through your pharmacist, who will in turn receive approval or denial from our office. Please allow 48-72 hours for refill requests to be processed.

Phone Calls

Phone calls will be returned as soon as possible. Please understand that our primary responsibility is to patients currently in the office, so messages will be returned between appointments or at the end of the day.
